A bill to amend title XVIII of the Social Security Act to improve the annual wellness visit under the Medicare program.

The Wellness and Education for Longer Lives for Seniors Act of 2025 (WELL Seniors Act) would broaden and strengthen Medicare’s annual wellness visit (AWV) under the Social Security Act. Key changes include expanding the set of elements that must be addressed during an AWV to cover nutrition, mobility, food and housing security, transportation access, social support, and other social determinants of health, plus a new screening for balance to assess fall risk. The bill also creates an enhanced payment incentive (an extra 10% of the AWV payment) for AWVs that include these required elements plus at least two additional specified elements, beginning January 1, 2026. In addition, the bill would include the AWV as a covered telehealth service, extend provider eligibility to physical therapists, occupational therapists, and pharmacists to furnish AWVs, require Education and Outreach activities (including a national campaign), and require guidance revisions, research, and focus group work to improve utilization. The act also directs ongoing education, outreach, and data analysis to inform improvements, with appropriations authority through 2030.
